Vernon College must have a signed copy of this form on file before you will be allowed to receive credit for these classes. To save yourself time during registration, please follow the instructions below:

• If you have a printer, proceed as follows:
  With the Concurrent or Dual Credit Enrollment Form page displayed on your screen, click on the printer icon. If you do not have a printer icon, left click on file then left click on print. Either method should direct a copy of the form to your printer. Follow the normal procedure for your printer.

• If you do not have a printer or if you are unable to print the Concurrent or Dual Credit Form, request a copy from Admissions & Records. Be sure to include your name and address.

• Complete the Dual Credit Enrollment Form and get the appropriate signatures. Mail to:
