displayed
on your
screen,
click on
the
printer
icon. If
you do
not have
a
printer
icon,
left
click on
file
then
left
click on
print.
Either
method
should
direct a
copy of
the form
to your
printer.
Follow
the
normal
procedure
for your
printer.

Questions
Form,
request
a copy
from
Admissions
&
Records.
Be sure
to
include
your
name and
address.

Complete
the
Residency
Core
Question
Form,
sign and
date the
Oath. If
you plan
to pay
the
$10.00
application
for
admission
fee by
check or
money
order,
attach
the
payment
to the
Residency
Core
Question
Form.
